Job summary

Center is seeking a Qualified Mental Health Professional to perform routine case management under the supervision of the Program Manager of Adult Mental Health in Denton County. The role emphasizes continuity of care, crisis intervention when necessary, and collaboration with a multidisciplinary team to address medical, social, and educational needs.

You will complete assessments (ANSA, Recovery Plan, CSSRS, etc.), coordinate services, and ensure documentation aligns with TAC and HHSC standards.

Qualifications

• Knowledge of community resources, case management delivery systems, and program policies and procedures.
• Ability to assess clients’ needs, coordinate services, communicate effectively, and train others.
• Ability to work in an interdisciplinary team and make decisions with guidance when needed.
• Trauma-informed care principles should guide practice and interactions.

Responsibilities

• Provide case management and related services; caseload size varies.
• Offer transportation and community base visits as required for LOC assignments.
• Ensure charting and documentation meet HHSC contract requirements; respond to audits.
• Support zero suicide initiative; connect individuals to appropriate programs.
• Maintain professional conduct and timely completion of required paperwork and duties.
• Engage independently while seeking guidance when needed.
