Denton County MHMR is seeking a qualified QMHP to perform moderately complex case management within the MCOT program in Denton, TX. You will ensure ROH assessments are followed up within 24 hours and help connect individuals to needed services.

Responsibilities include coordinating care, maintaining accurate records, and working with a team to provide trauma-informed crisis interventions. A four-year degree in a human service field and QMHP credential are required.

Formación

• Bachelor's degree in a human service field required.
• QMHP credential and crisis experience preferred.

Responsabilidades

• Ensure 24hr follow-up after ROH assessments and connect clients to services.
• Provide case management and collaborate with MCOT for appropriate LOC levels.
• Maintain caseloads, document care, and coordinate with supervisors.
