Job summary

Children's Aid Society in New York seeks a Therapeutic Case Planner to deliver trauma-informed, strengths-based services to families affected by IPV.

You will conduct safety-focused assessments, develop individualized intervention plans, provide supportive counseling, and advocate to help children thrive in safe, stable homes.

This role reports to the Program Supervisor, requires an MSW, and involves coordinating with ACS and community partners to reduce barriers to services.

Qualifications

  • Master's degree in social work is required.
  • Experience with trauma-informed care preferred.
  • Experience with NYC ACS and child welfare families preferred.

Responsibilities

  • Maintain a caseload of approximately 10-11 families and conduct two face-to-face contacts monthly.
  • Conduct joint home visits with ACS and explain program requirements and benefits.
  • Administer the ACS Domestic Violence Screening Tool and complete intake/assessment documentation.
  • Provide crisis intervention, counseling, safety planning and advocacy.
  • Develop and maintain Family Assessments and Service Plans with measurable goals.
  • Maintain accurate case records in PROMIS and Connections databases.
  • Coordinate with community-based providers and make referrals.
  • Attend Family Team Conferences and supervision meetings.
