Good Shepherd Services in Brooklyn, NY seeks a Case Planner to provide home-based and on-site counseling to a designated caseload of eight families, employing strength-based, trauma-informed practices.

You will engage families upon referral, coordinate with ACS and other partners, implement SBC processes, and document outcomes to support child welfare goals. Evening availability is required to accommodate families and ensure ongoing services.

Qualifications

• Bachelor’s degree required in Social Work, Human Services, or related field.
• Experience in child welfare preferred.
• Strong organizational, interpersonal, and counseling skills.
• Ability to work at least 2 evenings per week.
• Commitment to strength-based, trauma-informed practice.

Responsabilités

• Provide home-based and on-site individual, family, and group counseling, plus short-term crisis intervention.
• Engage families upon referral and participate in ACS Child Safety Conferences when possible.
• Implement SBC principles: assessment, outcomes, action plans, documentation.
• Handle crises with professionalism; report to the State Central Registry as needed.
• Screen and assess substance use, mental health, and child safety and well-being.
• Attend ACS and GSS meetings and trainings.
