Qualifications

• Experience delivering trauma-informed coaching and case management.
• Ability to conduct home visits and manage documentation.
• Familiarity with housing stability, education, and employment goals.

Responsibilities

• Provide individualized coaching to 18 families in supportive housing.
• Develop service plans and maintain required documentation.
• Coordinate services with internal and external partners and respond to crises.
