Job summary

New Moms is seeking a Housing Coach to provide individualized, trauma-informed coaching and case management for families in our Oak Park supportive housing program. You will maintain a caseload of about 18 families, helping parents reach housing stability, employment, education, and family goals.

Based at our Oak Park Main Office, this full-time role requires on-site presence, strong collaboration with staff and external partners, and occasional after-hours support.

Qualifications

• Bachelor’s in social work, psychology, counseling, or similar field required.
• OR Bachelor’s in another field plus 2 years clinical/human service experience.
• OR High school diploma plus 5 years clinical or human service experience.

Responsibilities

• Partner with 18 families to provide individualized, trauma-informed coaching and case management focused on housing stability, employment, education, and family well-being.
• Conduct assessments, develop service plans, complete IM+CANS updates, and maintain participant documentation on time.
• Conduct regular in-person home visits to support housing transitions, eviction prevention, and crisis situations.
• Connect participants to community resources and coordinate services with staff and external partners.
• Lead monthly house meetings and ongoing supportive communication to build participant engagement.
• Participate in after-hours on-call rotation to support participant emergencies.
• Maintain IM+CANS certification and contribute to the New Moms Coaching Model and professional development.
• Supervise volunteers or interns as needed and participate in staff meetings and workgroups.

Position Summary

The Housing Coach provides individualized coaching and family support to families living in our 18-unit supportive housing program in Oak Park. This position maintains a caseload of approximately 18 families, helping parents achieve goals related to housing stability, education, employment, and family well-being. Using a trauma-informed, Housing First approach, this role provides coaching, case management, resource connection, advocacy, and crisis support to help families build stability and thrive.

Location: Main Office in Oak Park, IL (206 Chicago Ave)

Schedule: Full-time, 40 hours/week

Starting Salary: $49,000 - $51,000

What a Day Might Look Like

No two days are exactly the same. A typical day might include:

• Meeting with a family in their home to check in on goals, provide coaching, and address emerging needs
• Helping a participant navigate a housing, employment, education, or family-related challenge
• Connecting families with community resources and coordinating services with internal and external partners
• Updating service plans, completing assessments, and documenting participant interactions
• Collaborating with colleagues to address housing stability concerns or prevent a potential crisis
• Supporting a participant experiencing emotional distress using a trauma-informed approach
• Responding to participant needs through New Moms' goal tracking platform and other digital coaching tools
• Participating in team meetings, professional development, or case consultation
• Facilitating an evening house meeting or community-building activity with residents
• Providing after-hours support as part of the Housing team's rotating on-call schedule

Key Responsibilities

• Partner with 18 families, providing individualized, trauma-informed coaching and case management focused on housing stability, employment, education, and family well-being
• Conduct assessments, develop service plans, complete IM+CANS updates, and maintain accurate participant documentation within required timelines
• Conduct regular in-person home visits and support families through housing transitions, eviction prevention, crisis situations, and other challenges
• Connect participants to community resources and coordinate services with New Moms staff and external partners
• Build participant engagement and community through monthly house meetings, activities, and ongoing supportive communication
• Participate in the Housing team's rotating on-call schedule to provide after-hours support for participant emergencies
• Maintain IM+CANS certification and participate in the New Moms Coaching Model and ongoing professional development
• Participate in staff meetings, agency initiatives, and workgroups; supervise volunteers or interns as assigned
• Maintain timely communication, uphold New Moms' values and service model, and perform other related duties as assigned

Qualifications

Education (one of the following required):

• Bachelor’s in social work, psychology, counseling, or similar field
• OR Bachelor’s in other field + 2 years clinical/human service experience
• OR High school diploma + 5 years clinical or human service experience

Plus:

• At least 1 year experience in a similar role preferred
• Valid driver’s license, daily access to a car, and insurance required to complete essential functions of job description
• Comfort using digital tools and case management software
• Ability to work one evening per week; occasional weekends

Work Environment:

Must be able to sit/stand for up to 8 hours, walk stairs, and lift up to 30–40 lbs (or perform with reasonable accommodation). Work environments may vary.
