Job summary

San Diego LGBT Community Center in San Diego is seeking a Karibu Case Manager to provide case planning, assessments, and crisis support for participants of the Karibu Permanent Supportive Housing Program. You will work with a trauma-informed team to help residents facing housing challenges achieve stability and independence.

The role involves conducting intakes, coordinating services, organizing community events, and maintaining accurate records in HMIS.

Qualifications

• 1 year of experience providing case management services to people experiencing homelessness or at risk of homelessness.
• Fluency in English.
• Proficiency with Microsoft Office.
• Ability to manage multiple tasks with shifting priorities.
• Competence in working with diverse populations.
• Lived experience from under-served communities encouraged.

Responsibilities

• Conduct intakes, assessments, and evaluations with clients.
• Provide links to mental health services, substance abuse services, educational services, career development services, and other relevant resources.
• Provide ongoing case management, crisis intervention, conflict resolution, and stabilization services to participants enrolled in the program.
• Collaborate with internal and external providers to support client care plans.
• Assist clients with adherence to housing quality standards and attend housing unit inspections.
• Organize community building events and coordinate workshops that promote client well-being and self-sufficiency
• Complete individualized service plans and self-sufficiency tools.
• Maintain accurate documentation of client's goals and needs, case management plans, and service entries into internal data systems.
• Enter program data and outcomes into HMIS to adhere to grant and contract compliance.
• Document thorough case notes that accurately reflect client progress toward individualized case management goals.
• Assist with the completion of monthly narrative reports for funder reports.
