Job summary

YMCA is seeking a Social Work Case Manager to provide trauma-informed, client-centered case management for LGBTQ+ youth under 21 in San Diego County. You will collaborate with the Y TAY team, deliver services in a drop-in setting, and coordinate with partners to promote self-sufficiency and life skills.

The role involves in-person work at the Escondido Center with some travel to Oceanside, and requires compliance with licensing, background checks, and safety trainings.

Qualifications

• Bachelor’s degree or equivalent work experience required
• Knowledge or experience of social work practices to individuals and families
• Basic computer knowledge and skills

Responsibilities

• Assess participants and plan case management with collaboration from participants and agencies
• Conduct screenings and assess service needs in various settings
• Maintain concise, well-written case notes and organized records for all participants
• Transport participants as needed and drive safely in YMCA vehicles
• Ensure adherence to YMCA policies, procedures and applicable laws
• Attend required staff meetings and trainings
