Job summary

The San Diego LGBT Community Center is seeking a Karibu Case Manager to deliver individualized case planning and supports to participants in the permanent supportive housing program. The role works within a trauma-informed team to improve housing stability and self-sufficiency for clients.

Responsibilities include intake assessments, crisis intervention, and collaboration with service providers, with emphasis on serving diverse LGBTQ+ communities and those experiencing homelessness.

Qualifications

• 1 year of experience providing case management services to people experiencing homelessness or at risk of homelessness.
• Fluency in English required.
• Must be comfortable in PC/Windows environments; proficient with Microsoft Office.
• Experience with strength-based, trauma-informed, and Motivational Interviewing techniques.
• Diversity experience encouraged; lived experience is valued.

Responsibilities

• Conduct intakes, assessments, and evaluations with clients.
• Provide links to mental health, substance abuse, educational, and career services.
• Provide ongoing case management, crisis intervention, and stabilization services.
• Collaborate with internal and external providers to support client care plans.
• Assist clients with adherence to housing quality standards and attend inspections.
• Organize community-building events and coordinate workshops.
