Job summary

UCAN Chicago is seeking a Case Manager to support foster care youth and families in Chicago. You will provide direct casework services, facilitate trauma-informed supports, and promote positive youth development under UCAN’s clinical philosophy.

Role requires a 4-year degree, 0–2 years of related experience, and exempt status. You will collaborate with DCFS, courts, and service providers to plan and monitor client progress and permanency options.

Qualifications

• SW or BS/BA in Human Service field is required.
• Licensed Child Welfare Services Employee or license eligible is required.
• 0-2 years of experience in human services, child welfare, or crisis intervention is required.

Responsibilities

• Completes assessments and service plans addressing client/family needs.
• Implements service plans, monitors progress, and reviews with supervisor/DCFS.
• Maintains accurate client records and reports as required by UCAN and IDCFS.
• Contacts courts, DCFS, and providers to coordinate client services or referrals.
• Plans, coordinates, and supervises required family or sibling visits.
• Demonstrates professionalism and holds appropriate boundaries with clients.
