Job summary

UCAN Chicago is seeking a Case Manager to support youth and families affected by gun and street violence in Chicago's West Pullman area.

The role provides crisis intervention, case management, advocacy, and collaboration with VIPS and the UCAN team to promote healing through trauma-informed services and Positive Youth Development.

Candidates should have a BSW/BA in a human service field, strong interpersonal skills, and willingness to work flexible hours, including evenings and weekends.

Qualifications

• BSW or BA/BS in a human service field or relevant experience.

Responsibilities

• Provide direct case management and emergency support to participants and families in community, hospitals, and in-office.
• Engage with partner agencies to create a network of support for individuals and families affected by violence.
• Coordinate and facilitate activities for workshops and groups for program participants.
• Maintain in-person contact with participants weekly, at minimum.
• Assist participants in accessing resources such as Victim’s Compensation and mental health services.
