UCAN in Chicago is seeking a Case Manager in Foster Care to provide direct, trauma-informed casework and support to youth and families. You will help with planning, service coordination, and empowerment for youth toward independence.

The role emphasizes safety, permanency planning, community resource linkage, and professional communication with courts and DCFS, while upholding UCAN's values and standards.

Qualifications

• SW or BS/BA in Human Service field required.
• Licensed Child Welfare Services Employee or license eligible.
• 0-1 years of experience working in human services, child welfare, or crisis intervention.

Responsabilités

• Completes assessments and service plans addressing client/family needs.
• Implements service plans, monitors progress, and reviews with supervisor.
• Assesses safety and permanency options and referrals for placement.
• Develops knowledge of community resources and maintains relationships with providers.
• Makes frequent progress reports and maintains client records.
