Job summary

Pinnacle Family Services is part of Health Connect America, a COA-accredited family of behavioral health brands. The Family Centered Treatment (FCT) Specialist delivers evidence‑based services in homes and communities, coordinating with a multi‑disciplinary team to support children and families on their path to wellbeing.

FCT Certification is required and will be supported by the organization with trainings and structured checkoffs.

Qualifications

• Master's in human services field + 1 year experience or
• Bachelor's in human services field + 2 years experience or
• Bachelor's in non-human services field + 4 years experience or
• Current NC license (including provisional) or board‑issued certification in a human services profession or
• Experience must be full‑time and supervised, serving children/adolescents with a primary mental health diagnosis

Responsibilities

• Deliver Family Centered Treatment services primarily in home and community settings, meeting families where they are.
• Guide children and families through the four phases of the FCT model, helping them set goals, build skills, and move toward sustainable well‑being.
• Serve as a central point of coordination for each family's Treatment Team, collaborating with medical, behavioral health, school, and community partners to ensure seamless, whole-person care.
• Develop, implement, and continuously refine individualized Service Plans in partnership with the families you serve.
• Connect families to community resources, referrals, and aftercare supports that extend the impact of treatment beyond your time together.
• Maintain timely, accurate documentation in the Electronic Health Record, keeping records that reflect the quality of care you provide.
• Participate in Quality Assurance activities that help make our programs stronger for every family we serve.
