Job summary

Pinnacle Family Services, part of Health Connect America, seeks clinicians to deliver Family Centered Treatment in home and community settings across North Carolina. You’ll work with children and families, guide them through the FCT model, and coordinate with the Treatment Team to ensure holistic, person-centered care.

FCT Certification is required within one year of hire. We provide structured checkoffs, e-learning trainings, and a supportive team to help you grow.

Qualifications

• Must meet NC Qualified Professional (QP) requirements per 10A NCAC 27G .0104.
• Current NC license or board-issued certification in a human services profession.
• Experience must be full-time and supervised, serving children and adolescents with a primary mental health diagnosis.
• Must have a valid driver’s license and access to transportation.

Responsibilities

• Deliver Family Centered Treatment services primarily in home and community settings, meeting families where they are.
• Guide children and families through the four phases of the FCT model, helping them set goals and build skills.
• Serve as a central point of coordination for each family's Treatment Team, collaborating with medical, behavioral health, school, and community partners to ensure seamless, whole-person care.
• Develop, implement, and continuously refine individualized Service Plans in partnership with the families you serve.
• Connect families to community resources, referrals, and aftercare supports that extend the impact of treatment beyond your time together.
• Maintain timely, accurate documentation in the Electronic Health Record, keeping records that reflect the quality of care you provide.
• Participate in Quality Assurance activities that help make our programs stronger for every family we serve.
