Job summary

Pinnacle Family Services, part of Health Connect America, is hiring a Family Centered Treatment (FCT) Specialist to deliver evidence-based services in homes and communities across North Carolina. You will work within a Treatment Team to support children and families with trauma-informed care.

FCT Certification is required within one year of hire. The role offers a flexible, community-based schedule with on-call crisis support and a pathway to professional growth within a trusted network.

Qualifications

• Must meet NC Qualified Professional requirements (10A NCAC 27G.0104).
• Master’s in human services field + 1 year experience.
• Bachelor’s in human services field + 2 years experience.
• Bachelor’s in non-human services field + 4 years experience.
• Current NC license or board-issued certification in a human services profession.
• Experience must be full-time and supervised, serving children and adolescents with a primary mental health diagnosis.

Responsibilities

• Deliver Family Centered Treatment services primarily in home and community settings, meeting families where they are.
• Guide children and families through the four phases of the FCT model, helping them set goals, build skills, and move toward sustainable well-being.
• Serve as a central point of coordination for each family’s Treatment Team, collaborating with medical, behavioral health, school, and community partners to ensure seamless, whole-person care.
• Develop, implement, and continuously refine individualized Service Plans in partnership with the families you serve.
• Connect families to community resources, referrals, and aftercare supports that extend the impact of treatment beyond your time together.
• Maintain timely, accurate documentation in the Electronic Health Record, keeping records that reflect the quality of care you provide.
• Participate in Quality Assurance activities that help make our programs stronger for every family we serve.
