Job summary

StepStone Family & Youth Services seeks a compassionate Community Support Specialist in Fort Wayne, IN to provide home-based case management, parenting education, and skills development for individuals and families. You will connect clients with community resources and support treatment goals with trauma-informed, strengths-based approaches.

You will document progress, collaborate with care teams, and help clients gain stability and independence in their homes and communities.

Qualifications

• Bachelor's degree in social work, psychology, marriage and family therapy, or related human service field.
• Previous experience preferred.
• Valid Driver's License in good standing and reliable transportation & auto insurance that complies with state law.
• Successful clearance of local, state, and national background checks.

Responsibilities

• Provide home base case management services.
• Provide Parenting Education.
• Understand and implement evidenced based moralities.
• Complete Casework Plan, weekly case notes, and monthly progress reports.
• Attend Supervision/Staffing bi-weekly.
• Facilitate and supervise family visitations.
• Assess family functioning and parent skills.
• Other duties as assigned.
