Responsibilities

• Provide home-based case management services and supports.
• Deliver parenting education and skills-building.
• Implement evidence-based practices where applicable.
• Complete casework plans, weekly case notes, and monthly progress reports.
• Attend supervision/staffing bi-weekly.
• Facilitate and supervise family visitations.
• Assess family functioning and parent skills.
• Perform other duties as assigned.
