Job summary

Pinnacle Family Services, part of Health Connect America, seeks a dedicated case manager to support youth and families across South Carolina. You will manage a dynamic caseload, engage children and caregivers, and tailor evidence-based interventions to achieve safety and wellness goals.

Qualifications

• Bachelor's degree in a related field (social work, psychology or behavioral science).
• Valid driver’s license, auto insurance and access to a vehicle.

Responsibilities

• Manage an assigned caseload and provide services per referrals and objectives.
• Engage children, families, and caregivers in service delivery and planning.
• Assess needs and identify interventions to support safety and outcomes.
• Plan and adjust interventions using evidence-based approaches.
• Maintain regular meetings with clients per program requirements.
• Travel as needed and document progress thoroughly.
