Health Connect America, Inc. (Pinnacle Family Services) is seeking a dedicated professional to manage a dynamic caseload and provide trauma-informed behavioral health services to youth and families in South Carolina.

You will collaborate with supervisors to schedule flexible hours, including evenings and weekends, to meet client needs while ensuring high-quality care. You will engage children and families in treatment planning, perform assessments, and coordinate with referring authorities.

Formación

• Bachelor's degree in a related field of social work, psychology or behavioral science.
• Valid driver’s license, auto insurance and access to a vehicle.

Responsabilidades

• Manage an assigned caseload of referrals and provide services in accordance with referral requirements, assessments, and established objectives.
• Engage children, families, and caregivers in service delivery and treatment planning.
• Assess client and family needs and identify appropriate interventions to support safety and desired outcomes.
• Plan, implement, and adjust interventions using evidence-based and best-practice approaches.
• Meet with designated clients for the prescribed number of hours, days, or contacts per week in accordance with program requirements.
• Maintain ongoing communication and coordination with referring authorities and involved stakeholders.
• Attend court hearings and provide testimony as required.
• Master professional documentation for progress tracking and quality of care.
• Transport clients using a personal vehicle, as necessary and in accordance with policy.
• Provide mentoring, coaching, and job shadowing to new or current staff.
