Health Connect America is seeking dedicated professionals to join its foster care and behavioral health teams in a trauma-informed, relationship-focused environment. You will work within a network of brands delivering evidence-based services to at-risk youth and families across nine states, with strong support and pathways for growth.

Qualifications

• Bachelor's degree in a related field of social work, psychology or behavioral science.
• Valid driver’s license, auto insurance and access to a vehicle.

Responsabilités

• Manage an assigned caseload of referrals and provide services in accordance with referral requirements, assessments, and established objectives.
• Engage children, families, and caregivers in service delivery and treatment planning.
• Assess client and family needs and identify appropriate interventions to support safety and desired outcomes.
• Plan, implement, and adjust interventions using evidence-based and best-practice approaches.
• Attend court hearings and provide testimony as required.
• Maintain professional documentation and coordinate with referring authorities and involved stakeholders.
• Transport clients using a personal vehicle, as necessary.

Formation

Bachelor's degree in a related field of social work, psychology or behavioral science
