Good Shepherd Services in Brooklyn offers a Case Planner role within the Brooklyn Transitions Program to provide home-based counseling and SBC-driven case management for families facing substance use or mental health challenges.

The position emphasizes engagement with eight families, crisis intervention, documentation, and collaboration with ACS and partner agencies while upholding a strength-based and trauma-informed practice culture.

Qualifications

• Bachelor’s degree is required.
• Child welfare experience is preferred.
• Must have strong organizational, interpersonal, and counseling skills.
• Must be able to work at least 2 evenings per week to accommodate working families’ schedules.
• Must have strong inquiry skills and ability to seek and share internal and external best practices.
• Commitment to work from a strength-based, trauma-informed & youth and family development perspective.

Responsibilities

• Provide home-based and on-site individual, family, and group counseling, including short-term crisis intervention.
• Engage families upon referral, including ACS Child Safety Conferences when needed.
• Implement SBC principles: assessment, outcomes, action plans, and documentation.
• Handle case-related crises with professionalism and urgency; reports to State Central Registry as needed.
• Conduct substance use, mental health, and child safety assessments; evaluate caregiver capacity and home conditions.
• Represent participant needs at ACS and GSS meetings and trainings.
• Collaborate with ACS, service providers and schools to ensure high quality services.
• Prepare and maintain FASP documentation and monthly reports.
