Job summary

Wheeler Clinic in Hartford, CT seeks an Early Childhood Specialist to support children ages 0-5 and their families. You will work with medical staff to provide developmental screening, care coordination and trauma-informed guidance.

This role requires a Master’s degree and extensive knowledge of early childhood development and family systems, plus a valid driver’s license. Fee-for-service, up to 28 hours weekly, at $27 per hour.

Qualifications

• Master's degree as specified is required.
• Extensive knowledge in early childhood development, emotional and behavioral health, family systems, assessment, attachment/separation issues, substance abuse, domestic violence, and abuse and neglect is also required.

Responsibilities

• Conducts team based well child visits with parents and their babies and young children with the physician/pediatric nurse practitioner.
• Provides consultations with families regarding developmental and behavioral concerns such as sleep, discipline, picky eating, etc.
• Collaborates with Healthy Steps team to implement program core components and ensure resources are available to practice staff.
• Provides consultation to medical professionals re: early childhood development, infant mental health, and trauma informed practice.
• Provides referrals and tracks follow-up to connect families to community resources.
• Maintains databases to meet Healthy Steps fidelity metrics including referrals.
• Completes annual site reporting to the National Office using EMR data reports.
• Documents patient clinical activity and care coordination in EMRs.
• Maintains communication with medical staff and care team.
