Job summary

Unbound Now in Texas is seeking a CSEY Care Coordinator to implement the Texas Model for Care Coordination, working with county partners and CSTT guidance to connect youth survivors to trauma-informed, holistic services.

The role requires building consensus, coordinating meetings, maintaining documentation in the electronic case management system, and participating in 24/7 crisis coverage with other coordinators. Work performed primarily remotely with some on-site requirements.

Qualifications

• Bachelor’s degree in a related field or equivalent experience.
• Trauma-informed, youth-centered, strengths-based practices in supporting youth.
• Experience collaborating with multidisciplinary teams (MDTs) and community partners.
• Ability to facilitate meetings, group discussions, and trainings.
• Excellent verbal and written communication skills in sensitive environments.

Responsibilities

• Build consensus among county partners to establish Care Coordination protocols.
• Implement Care Coordination in line with CSTT guidance and grant requirements.
• Participate in 24/7 remote crisis coverage with other Care Coordinators.
• Complete CSE-ITs and support responses based on screening results.
• Document referrals, meetings, and action plans promptly in the case management system.
