Ada S McKinley Community Services Inc. seeks a Crisis Continuum of Care Community Based Clinician (CBC) in Chicago to provide 90-day community-based behavioral health services, assessments, crisis intervention, stabilization, and linkage to ongoing care for individuals and families in crisis.

Under LPHA supervision, the CBC delivers trauma-informed, recovery-oriented, culturally responsive services across homes, schools, hospitals, and community settings, coordinating with MRSS, CSP, and the 590

Qualifications

• Bachelor's degree in Psychology or related field.
• QMHP preferred with a Master’s degree.

Responsibilities

• Performs comprehensive, in-depth assessment and clinical formulation, development of accurate diagnosis and strengths-based treatment plans for Crisis Continuum of Care services.
• Provides a full range of case management, counseling, advocacy, and educational services.
• Identifies and utilizes internal agency and external community resources to meet client needs.
• Assess broader community conditions impacting clients to identify systemic issues and resource gaps.
• Provides group-based services as indicated by program needs.
• May be responsible for assisting with 24-hour crisis coverage.
• Represents and interprets agency programs to the community, fostering collaboration and partnership.
• Supports and attends community events, including initiatives to increase safety and community cohesion.
• Establishes and maintains a strong referral network with outside agencies to ensure continuity of care for participants and their families.
• Takes a proactive role in the development and enhancement of services and programming that integrate trauma-informed care principles.
• Meets agency expectations for quantity and quality of direct service delivery.
• Completes all required agency documentation and record-keeping in a timely and accurate manner.
• Works effectively and respectfully with a diverse staff and participant population.
• Other duties as assigned.
