Qualifications

• Requires a bachelor’s degree in a human services field or equivalent combination of education and experience.
• Experience delivering services to individuals with mental illness, substance use disorders, or developmental disabilities.
• Ability to collaborate with clients to develop and update person-centered plans.
• Ability to complete documentation within timelines and meet quality standards.

Responsibilities

• Collaborates with clients to develop individualized person-centered plans and reviews/updates to guide ongoing treatment services.
• Effectively monitors and assesses clients’ participation and progress in treatment according to the plans.
• Provides treatment interventions that are medically necessary and ethical per client plans.
• Communicates relevant clinical information with supervisor and key staff regarding clients’ needs promptly.
• Documents client safety planning and hospitalization interventions per guidelines.
• Ensures all required documentation is completed within timelines and quality standards.
• Meets or exceeds billing and productivity expectations as established by program leadership.
• Supports coworkers in providing trauma-informed, person-centered services and collaborates with the team.
• Completes training and HR compliance documents timely.
• Helps clients obtain and maintain medical insurance benefits and community resources.
