Job summary

Integrated Services for Behavioral Health in Ohio is seeking a Community Behavioral Health Worker to provide coaching and practical support in home, school, and community settings, helping clients develop routines, coping skills, and healthy communication.

The CBHW guides access to community resources, collaborates with internal teams and external partners, uses trauma-informed, strengths-based approaches, and travels within the designated service areas to deliver care.

Qualifications

• High School diploma or GED required.
• Bachelor’s degree or 3+ years direct relevant experience preferred.
• Experience with multi-need individuals and families is required.
• Knowledge of community service systems is preferred.

Responsibilities

• Engage individuals and families in home and community to identify strengths, barriers, and goals related to behavioral health and daily functioning.
• Provide skill building, coaching, and practical supports in home, school, and community settings.
• Assist clients in navigating community resources through modeling, coaching and teaching.
• Deliver care in home and community-based settings.
• Travel within the designated service areas.
• Collaborate with internal programs and external partners to ensure quality of care.

Education
