Integrated Services for Behavioral Health (ISBH) in Ohio seeks a Community Behavioral Health Worker to support individuals, families, and communities through home- and community-based behavioral health services. Qualifications

• Minimum three years of relevant experience or a Bachelor’s degree.
• High School Diploma.
• Demonstrated cultural awareness.
• Experience with multi-need individuals and families.
• Broad knowledge of community service systems.
• Licensed and able to operate a motor vehicle with appropriate insurance.
• Willing to participate in and lead cross-systems team-building activities.
• Able to communicate effectively verbally and in writing.
• Ability to operate in an Internet-based, automated office environment.

Responsibilities

• Assesses or defines strengths and needs of referred people, families, and communities.
• Directly provides home and community behavioral health services that are practical, helpful, and collaborative.
• Meets people wherever they may be with support for self-management of health and well-being.
• Carries out utilization review and quality assurance activities as directed.
• Maintains documentation, participates in program evaluation, attends meetings, and learns about community resources.
• Meets billing productivity requirements established by ISBH.
• Supports growth in neighboring counties.
• Other duties as assigned.
