Job summary

Integrated Services for Behavioral Health (ISBH) seeks a Community Behavioral Health Worker to deliver in-home and community-based supports across Southeast and Central Ohio. This role emphasizes trauma-informed, strength-based care and collaboration with clients and families to promote recovery and resilience.

The position involves coaching, resource navigation, and documentation in EHR systems, with travel within designated service areas.

Qualifications

• HS diploma or GED required.
• Bachelor's degree or 3+ years direct relevant experience preferred.
• Experience with multi-need individuals and families preferred.
• Valid driver’s license and reliable vehicle required.

Responsibilities

• Engage individuals and families in home and community to identify strengths, barriers, and goals related to behavioral health and daily functioning.
• Provide skill building, coaching, and practical supports in home, school, and community settings.
• Support clients in developing routines, coping skills, emotional regulation, executive functioning skills, and healthy communication patterns.
• Assist clients in navigating community resources through modeling, coaching and teaching.
• Deliver care in home and community-based settings.
• Collaborate with internal programs and external partners to ensure quality of care.
• Maintain consistent communication with clients, caregivers, and team members to support engagement and follow through.
• Utilize databases and EHR systems for accurate client documentation.
• Adhere to ISBH EHR documentation policy.
• Meet ISBH standards for productivity and quality of service delivery.
• Maintain compliance with HIPAA and mandated reporting requirements.
