Job summary

Integrated Services for Behavioral Health (ISBH) seeks a Community Behavioral Health Worker to provide in-home and community-based coaching and support. The role connects clients to resources, builds routines and coping skills, and collaborates with partners to promote recovery and resilience.

Trauma-informed, strengths-based approaches guide all interactions. Prior experience in behavioral health and the ability to travel within Washington County are required.

Qualifications

• A high School diploma or GED equivalent is required.
• A bachelor’s degree in related field or minimum 3 years of direct relevant experience is required.
• Experience with multi-need individuals and families is required.
• Experience partnering with local behavioral health systems, housing, legal systems, and social services is preferred.

Responsibilities

• Engage individuals and families in the home and community to identify strengths, barriers, and goals related to behavioral health and daily functioning.
• Provide skill building, coaching, and practical supports in home, school, and community settings.
• Support clients in developing routines, coping skills, emotional regulation, executive functioning skills, and healthy communication patterns.
• Assist clients in navigating community resources through modeling, coaching and teaching.
• Deliver care in home and community-based settings.
• Collaborate with internal programs and external partners to ensure quality of care.
• Maintain consistent communication with clients, caregivers, and team members to support engagement and follow through.
• Utilize EHR systems for accurate client documentation; adhere to HIPAA and ISBH policies.
