Job summary

Youth Homes seeks a compassionate Clinician and Intensive Care Coordinator (ICC) to provide evidence-based therapy and coordinate comprehensive care for youth, caregivers, schools, and community providers in Contra Costa County. You will develop collaborative care plans that reflect each family’s voice and connect youth with needed supports in a respected nonprofit with over six decades of service.

Qualifications

• Master's degree in a relevant field is required.
• Current California licensure (A/LMFT, A/LCSW, A/LPCC) is required.
• Commitment to trauma-informed, culturally responsive and family-centered care.

Responsibilities

• Provide strengths-based, trauma-informed mental health care and therapy for youth and families.
• Coordinate services across schools, providers, and community partners.
• Lead Child and Family Team meetings and ensure collaborative decision-making.
• Maintain timely, high-quality clinical documentation in compliance with Medi-Cal/HIPAA.
