Job summary

Qualifications

• Bachelor’s degree in social work or related field.
• Minimum of three years of experience working with children and families.
• MDHHS ICCW Certification preferred, or willingness to complete certification within 90 days of hire.
• Commitment to diversity, equity and inclusion.
• Valid driver’s license and ability to drive as needed.

Responsibilities

• Provide Intensive Care Coordination using ICCW to identify strengths, assess needs, and develop plans.
• Work primarily in the home or community, with occasional tele-health as appropriate.
• Maintain records in EMR per MDHHS and OCHN standards.
• Coordinate with child, family, therapists, schools and other supports to meet goals.
