Job summary

Advocate Health is seeking a full-time Trauma Recovery Services team member based at Advocate Illinois Masonic Medical Center in Chicago. You will support trauma survivors with immediate emotional care, safety planning, and discharge coordination, linking patients to outpatient resources and clinical teams.

The role requires strong communication, empathy, and the ability to work with diverse populations, including families.

Qualifications

• High School Diploma or GED required.
• Minimum one year experience in social services.
• Ability to work flexible shifts including nights and weekends.

Responsibilities

• Provide trauma-informed emotional support and advocacy to hospitalized patients.
• Coordinate resources and outreach with community teams to mitigate retaliation risks.
• Engage in intensive community outreach and follow-up for outpatient connections.
• Schedule and coordinate care with TRC and community partners to ensure ongoing treatment.
• Conduct intakes and needs assessments; document action plans and connect to services.
