Qualifications

• Bachelor's degree in Social Work, Behavioral Science, or Social Science required.
• One year related experience acceptable with a relevant bachelor's degree; higher degrees may substitute.
• Strong documentation and communication skills expected.

Responsibilities

• Perform case management in the Child Welfare area of Family Preservation per DSS policy and GPS Practice Model; provide time-limited interventions.
• Conduct ongoing safety assessments and match services to needs; monitor safety, risk and well-being.
• Complete and update FAST to collaboratively develop Family Permanency Plans and make timely referrals.
• Engage families and professionals with empathy; respond to calls, emails and texts; locate and engage absent parents and kin.
• Maintain case records per policy; document in CAPSS; ensure quality documentation reflecting safety and permanency goals.
• Collaborate with legal when safety concerns exist or legal action is needed; participate in proceedings.
