Elevance Health is seeking a CFSP Managed Care Coordinator in North Carolina to manage members’ individual care plans in the CFSP program. The role is field-based, requiring travel to client sites and occasional office attendance to support coordinated care.

You will work with county child welfare and a multidisciplinary team to ensure access to physical health, behavioral health, pharmacy, and LTSS services. The role emphasizes trauma-informed care, collaboration, and patient-centered outcomes.

Competenze

• BA/BS degree and at least 1 year of direct experience with the program population or related community organizations.
• Must reside in North Carolina.
• Two years of experience with child welfare preferred.
• Colocation may require reporting to office 1–3 days per week.
• Willingness to travel to worksites and other locations as needed.

• Provide integrated whole-person care management across physical health, behavioral health, and related services.
• Coordinate care with county Child Welfare Worker to align health needs with permanency goals.
• Conduct telephonic or face-to-face assessments using predefined questions.
• Identify members with potential needs and coordinate with clinical teams for care.
• Oversee non-clinical needs of members with chronic illnesses or disabilities.
• Set short- and long-term goals with members, caregivers and physicians.
