Elevance Health, Inc. seeks a CFSP Managed Care Coordinator to manage members? individual service plans within the NC CFSP Program, coordinating care across physical health, behavioral health, I/DD, LTSS, and pharmacy.

This field-based role operates primarily in Haywood County (Waynesville) with travel to client sites and occasional office attendance for meetings and training. Strong collaboration with county staff and a multidisciplinary team is essential.

Qualificações

• BA/BS degree required; 1 year of relevant experience or equivalent background.

Responsabilidades

• Provide integrated whole-person Care Management under the CFSP model across physical health, behavioral health, I/DD, LTSS, pharmacy, and unmet health needs.
• Collaborate with County Child Welfare Workers to align health care with permanency goals.
• Work with a multidisciplinary care team to coordinate care (e.g., juvenile justice awareness).
• Conduct telephonic or face-to-face assessments using predefined questions to identify and manage needs.
• Oversee non-clinical needs of members with chronic illnesses or disabilities for cost-effective service utilization.
• Set short- and long-term goals with members, caregivers, families, and physicians.
• Identify members who would benefit from expanded services.
