Job summary

Elevance Health seeks a CFSP Managed Care Coordinator to manage members' individual service plans within the NC CFSP Program. This field-based role travels to client sites and requires interaction with patients, members, or providers in person four to five days per week.

The role emphasizes integrated care across physical health, behavioral health, I/DD, LTSS, pharmacy, and unmet needs, with a focus on trauma-informed, patient-centered coordination. NC residency is preferred for this position.

Qualifications

• Requires BA/BS and a minimum of 1 year experience working directly with people in the program population or related community-based orgs.
• Two (2) years of experience with child welfare is preferred.
• Must reside in North Carolina.
• BA/BS preferred in health, psychology, sociology, social work, nursing or related human services field.

Responsibilities

• Provide integrated whole-person Care Management across CFSP domains (physical, behavioral health, I/DD, LTSS, pharmacy).
• Offer Trauma-Informed Care by recognizing ACEs and coordinating cross-agency care.
• Collaborate with the County Child Welfare Worker to align health care needs with permanency goals.
• Work with multidisciplinary care team including primary and behavioral health pros and specialists.
• Conduct telephonic or face-to-face assessments to identify and manage member needs.
• Identify members with potential clinical health care needs and coordinate with clinical teams.
• Oversee non-clinical needs of members with chronic illnesses or disabilities for cost-effective care.
• Set short- and long-term goals with members, families, and physicians.
• Identify members who would benefit from expanded services.
