Elevance Health, Inc. is seeking a CFSP Managed Care Coordinator to lead the member’s individualized care plans within North Carolina’s CFSP program.

This field-based role requires engagement at client sites, with in-person interaction four to five days a week to ensure seamless, coordinated care. You will collaborate with county child welfare workers and a multidisciplinary team to manage care across physical health, behavioral health, and support services, including trauma-informed approaches

Qualifikationen

• BA/BS degree required in health, psychology, social work, or a related field.
• Minimum 1 year of experience working directly with people served by the CFSP population or similar community-based programs.

Aufgaben

• Provide integrated care management under the CFSP model, coordinating across physical, behavioral health, and social support services.
• Collaborate with county Child Welfare Workers to align health needs with permanency goals.
• Work with a multidisciplinary care team to coordinate care across providers and services.
• Conduct telephonic or in-person assessments to identify and manage member needs.
• Monitor non-clinical needs of members with chronic illnesses or disabilities for cost-effective care.
