Job summary

Medical College of Wisconsin (MCW) seeks a trauma recovery center case manager to provide comprehensive, trauma-informed case management for survivors of violent crime. You will assess needs, coordinate services, and support engagement in care across medical, mental health, legal, and social services.

You will work with a multidisciplinary team to reduce barriers to care, promote stability, recovery, and access to resources, and ensure culturally responsive services.

Qualifications

  • Minimum education of an Associate's degree is required.
  • Preferred Bachelor’s degree in social work, psychology, public health, or related field.
  • Experience substitutable for education on an equivalent basis.

Responsibilities

  • Conduct assessments to identify client needs across housing, safety, medical, legal, and financial domains.
  • Collaborate with clients and clinicians to develop individualized service plans.
  • Monitor and update client needs and goals throughout services.
  • Provide ongoing case management to support client stability and recovery.
  • Connect clients to resources including shelter, housing, healthcare, legal advocacy, and benefits.
  • Assist clients with applications for victim compensation, benefits, and insurance.
  • Coordinate care across multiple providers and systems.
  • Engage clients through proactive outreach to ensure retention in services.
  • Maintain regular contact via phone, in-person, and community interactions.
