Job summary

Society of St Vincent de Paul in Phoenix, AZ, is seeking an IHC Case Manager - Ozanam Manor to support residents of shelter and transitional housing programs. The Case Manager provides trauma-informed, strengths-based services to help residents access housing, income, health, and stability.

This role requires strong communication, organization, and the ability to work independently and with staff, volunteers, and community partners; includes maintaining records and coordinating services.

Qualifications

• Bachelor’s degree in social work or related field preferred.
• High School diploma or GED required.
• Trauma-informed and strengths-based support experience preferred.
• Experience with vulnerable populations in shelter or transitional housing a plus.

Responsibilities

• Carry a caseload of residents, meeting with each resident at least weekly.
• Complete assessments and individualized service plans with residents to identify needs, strengths, goals, and barriers.
• Monitor and document progress toward stabilization, income, and housing goals using case notes and progress reports.
• Maintain timely and accurate records in data systems within 24 hours of interaction.
• Coordinate/participate in incident intervention and conflict resolution with staff and security as needed.
• Assist residents in identifying and navigating community resources, including referrals and hands-on support.
