Job summary

ICL inc. is seeking a dedicated Case Manager to join a trauma-informed rehabilitation team in Brooklyn.

The role focuses on client-centered care, developing treatment plans, and supporting adults and transition age youth with mental health needs and potential co-occurring substances use disorders. You will provide skills training, maintain records, and coordinate with mental health providers to support recovery and community reintegration.

Qualifications

• Minimum degree requirements in human services as specified.
• Experience in mental health/crisis environments preferred.
• Ability to work with diverse populations and document plans.

Responsibilities

• Secures information and develops consumer treatment plans including quarterly reviews.
• Provides supportive counseling in 1:1 or group formats.
• Maintains accurate case records and documents progress and reviews.
• Facilitates individualized services and discharge planning for consumers.
• Uses cognitive-behavioral techniques to aid in meeting service plan goals.
• Ensures medication compliance and monitors self-administration per policy.
• Accompanies consumers to medical and social appointments as needed.
• Performs crisis assessment and intervention using approved methods.
