You will coordinate ISP creation and ensure timely discharge planning while maintaining confidentiality and high-quality documentation.

The role requires a Bachelor's in social science and at least one year of case management experience, with strong collaboration with community agencies and proficiency in Microsoft Office.

Responsibilities
