Job summary

Community Solutions, Inc. (CSI) seeks a Case Manager to join the Watkinson House team in Hartford, CT. Responsibilities include intake assessments, individual and group counseling, case record management, discharge planning and coordinating aftercare services.

Schedule: Mon/Wed/Fri 9:00am–5:00pm and Tue/Thu 1:00pm–9:00pm. Pay rate: $25.95/hour. CSI values trauma-informed practice and collaboration with partner agencies to support clients' successful community reintegration.

Qualifications

• Bachelor's degree in a social science field and one year experience in case management or similar field; substitution allowed year-for-year.
• Excellent oral, written and interpersonal skills desired.
• Ability to engage and develop collaborations with community agencies.
• Previous assessment experience preferred.
• Experience with Microsoft Office.

Responsibilities

• Check daily for newly assigned clients.
• Complete assessments within 3 business days of intake.
• Facilitate feedback and creation of ISP within 5 business days of intake.
• Maintain and submit discharge plans 60 days before scheduled release.
• Record sessions and upload for quality assurance review.
• Assist clients with obtaining birth certificates and social security cards.
• Maintain confidential client records per agency policy.
