The role involves client assessments, service planning, resource coordination, and maintaining documentation to support housing stability and individualized goals. Strong collaboration with staff and community partners is essential.

Qualifications

• Minimum 1 year of case management or related client services experience.
• Experience serving individuals experiencing homelessness or behavioral health challenges preferred.
• Ability to work with trauma-informed, client-centered approaches.

Responsibilities

• Provide trauma-informed case management and supportive services.
• Conduct intake screenings, assessments, and service planning.
• Coordinate housing resources, referrals, transportation, and appointments.
• Develop and monitor individualized service plans and goals.
• Maintain accurate client documentation and HMIS entries.
• Collaborate with internal and external partners to support client needs.
