Job summary

SummitStone Health Partners in Fort Collins, CO seeks a Case Manager PREVAIL – Data Lead to provide intensive, community-based case management for adults and youth in a deflection program. You will coordinate outreach, assessments, goal planning, referrals, and ongoing support while collaborating with law enforcement, behavioral health providers, and community partners.

The role includes data integrity work, grant reporting, and compliance activities.

Qualifications

• Bachelor's degree in Behavioral Health or related field required.
• One year of experience in behavioral health or case management required.
• Ability to complete required trainings (GAIN, CiviCore, Motivational Interviewing, Mental Health First Aid, PREVAIL-related).

Responsibilities

• Provide intensive, community-based case management for PREVAIL participants.
• Coordinate outreach, enrollment, goal planning, referrals, and ongoing support.
• Document services and data in required systems for grant reporting and QA.
