Job summary

SummitStone Health Partners in Fort Collins, CO, is seeking a Case Manager PREVAIL – Data Lead to provide intensive, community-based case management for adults and youth in PREVAIL, focusing on behavioral health, housing, benefits, and employment.

The role involves coordinating outreach, enrollment, goal planning, referrals, and progress monitoring, while working with law enforcement, housing providers, schools, and community partners to promote stability.

Qualifications

• Bachelor's degree in Behavioral Health or related field required.
• At least one year of experience in behavioral health, case management, or a related human services field.
• Ability to complete required program trainings including GAIN, CiviCore, Motivational Interviewing, Mental Health First Aid, PREVAIL trainings.
• Valid Colorado driver's license and reliable transportation; local travel may be required.

Responsibilities

• Provide intensive, community-based case management for PREVAIL participants.
• Coordinate outreach, assessment, goal planning, referrals, and service linkages.
• Collaborate with law enforcement, behavioral health providers, schools, housing agencies, and community organizations.
• Maintain thorough documentation in required systems for grant reporting.
• Support program fidelity and grant compliance through trainings.
• Provide trauma-informed, harm-reduction, and person-centered services.
