Job summary

Qualifications

• Bachelor's degree in Behavioral Health or related field is required.
• One year of experience in behavioral health, case management, or related human services field is required.
• Professional proficiency in both English and Spanish is required (bilingual role).

Responsibilities

• Receives, reviews, and manages referrals from law enforcement, criminal justice partners, schools, families, and other sources; prioritizes appropriate PREVAIL referrals and outreach.
• Conducts participant outreach, enrollment support, goal planning, and monitoring of progress toward identified goals.
• Provides trauma-informed, harm-reduction, and person-centered intensive case management and service coordination across health, housing, benefits, and social supports.
• Maintains coordination with law enforcement, healthcare providers, housing providers, and community partners to support deflection and engagement.
• Documents services, outreach, assessments, referrals, goals, and case management contacts in required systems for grant data entry, reporting and QA.
