Job summary

SummitStone Health Partners in Fort Collins, CO seeks a full-time Case Manager to coordinate integrated services for adult and youth populations. You will engage clients, assess needs, plan goals, and connect them with behavioral health, housing, benefits, and community supports.

This role emphasizes trauma-informed, harm-reduction, and client-centered care. Qualified applicants hold a Bachelor’s degree in behavioral health or related field and have at least one year of relevant experience.

Qualifications

• Bachelor’s degree in behavioral health or related field required.
• One year of experience in behavioral health, substance use treatment, case management, or a related human services field required.

Responsibilities

• Receives, reviews, and manages referrals from law enforcement, co-response, criminal justice partners, community partners, schools, families, and other referral sources; prioritizes PREVAIL referrals and completes outreach and engagement activities.
• Conducts participant outreach, screening coordination, enrollment support, GAIN assessment coordination, individualized goal planning, and ongoing monitoring of progress toward identified goals.
• Provides trauma-informed, harm-reduction, and person-centered intensive case management services, including service coordination and linkage to behavioral health, housing, benefits, education, employment, and other supports.
• Documents all services, outreach attempts, assessments, referrals, participant goals, service linkages, incentives, and case management contacts in required systems.
