Job summary

H.O.M.E., Inc. seeks a Case Management Lead to oversee the Case Management and Housing Navigation team, delivering trauma-informed support to individuals and families experiencing homelessness or housing instability.

This role blends leadership with direct service oversight to ensure high-quality, coordinated care. You will supervise staff, manage service planning, engage with community partners, and help drive program evaluation and improvement while maintaining compliance with policies and

Qualifications

• Minimum of 3 years of case management or related human services experience.
• Commitment to trauma-informed practices.
• CPR & First Aid certification or willingness to obtain.
• Strong leadership and mentorship abilities.
• Excellent written and verbal communication skills.
• Ability to maintain accurate documentation.
• Valid driver’s license and reliable transportation.
• Comfort working in a fast-paced environment.

Responsibilities

• Supervise Case Management and Housing Navigation staff with coaching and feedback.
• Oversee service planning to ensure consistency and quality.
• Maintain a small caseload to stay connected to direct service needs.
• Collaborate with partners to coordinate care and expand resources.
• Ensure compliance with policies, confidentiality, and funding requirements.
• Lead team meetings and case reviews.
• Track outcomes and report on program evaluation.
• Serve as liaison between staff and senior leadership.
• Participate in strategic planning for new initiatives.
