Job summary

Omni Family of Services Tennessee Inc is seeking a Care Coordinator to work with clients and their families in home settings, coordinating care and connecting with community supports. Work hours generally do not exceed 40 per week, with some on-call and after-hours responsibilities.

In this role, you will engage with foster, adoptive, and biological families, supporting safety and positive outcomes through trauma-informed, family-centered care within our multidisciplinary team.

Qualifications

• Bachelor’s degree in a behavioral health field such as counseling, social work, or psychology.
• Valid driver’s license, proof of automobile insurance and reliable transportation.
• Certification in crisis prevention initiatives and universal precautions required.

Responsibilities

• Meet with clients and families in home or residence to assess needs.
• Coordinate services and advocate for clients across school, court, job sites and other supports.
• Maintain caseload with schedules not exceeding 40 hours per week; on-call and after-hours as needed.

Education
