Omni Family of Services Kentucky Inc seeks a Community Clinical Coordinator to provide direct services to clients and their families within the FITT program. You will support the FITT Director and coordinate care for youth at risk of out-of-home placement in a 90-day intensive program.

The role emphasizes trauma-responsive, evidence-based care, collaborative teamwork, and a commitment to safety, healing, and growth for families and communities.

Qualifications

• Masters degree in a social work related field required.
• Must be on a full independent billable licensure track under supervision.

Responsibilities

• Provide direct services to clients and families within the FITT program.
• Support the FITT Director and coordinate care for youth at risk of out-of-home placement.
• Engage in trauma-responsive, evidence-based treatment approaches.
